Scholarships for the 2026-27 Academic Year

This application form is for two separate scholarships, both administered by the Bleeding Disorders Foundation of North Carolina.

1. Optum® Infusion Pharmacy Scholarship
   1. Multiple awards of $500 to $3000
   2. Eligibility: Applicant may be any person affected by a bleeding disorder, including a family member of someone diagnosed with a bleeding disorder (see candidate criteria in the section below)
   3. Funded by Optum Infusion Pharmacy
2. George D. McCoy Education Scholarship
   1. $1000 award
   2. Eligibility: Applicant must be diagnosed with mild, moderate, or severe Hemophilia A
   3. Funded by Bleeding Disorders Foundation of North Carolina

Any applicant diagnosed with Hemophilia A is automatically considered for both scholarships.

Applicants must be seeking education from an accredited college, university, or certified training program based in the United States. At least one award will be made to an applicant pursuing education in a health related field.

Awards are primarily merit based, but financial need will be considered as well. A copy of the applicant's current FAFSA (Free Application for Federal Student Aid – available at fafsa.gov) is required. The FAFSA is a detailed analysis of how much an applicant can be expected to pay toward the cost of their education. All students expecting to enroll in undergraduate, graduate or professional, or technical programs are eligible to complete the FAFSA, even if they are not expecting to qualify for federal aid. If the applicant feels that the FAFSA report is not an accurate representation of their financial need, then they may submit additional information for consideration.

All personal information will be treated as completely confidential. The Bleeding Disorders Foundation of North Carolina (BDFNC) and Optum Infusion Pharmacy will never divulge the applicant's name, address, financial, or health-related information to any other organization, company, or individual without their express permission. Scholarship winners may be asked (though not required) to submit a photograph and sign a release form for publicity purposes.

Applicants are not required to be—nor is any preference given to—residents of North Carolina.

Optum® Infusion Pharmacy Scholarship

Candidate Criteria: Scholarships are intended for persons affected by a bleeding disorder*, including anyone who

1. has been diagnosed with a bleeding disorder; or
2. is a caregiver of a child or adult affected by a bleeding disorder; or
3. has a sibling or parent in the same household affected by a bleeding disorder.

*For the purpose of this scholarship application, the term "bleeding disorder" is limited to clotting factor deficiency disorders, specifically: hemophilia A (factor VIII), B (factor IX), and C (factor XI), von Willebrand Disease, factor V/parahemophilia**, factor VII, factor X, and factor XIII deficiencies. (**Factor V Leiden is a clotting disorder and is not eligible). Exceptions for bleeding disorders that result from other rare diseases may be considered by the scholarship committee on a case-by-case basis.

Application instructions

Requirements: To complete this application you will need to enter personal demographic information and upload several documents. Please be prepared to submit all of the following documents in PDF format:

1. A physician's letter confirming the affected family member's bleeding disorder.
2. High school and/or college transcripts.
3. Essay: Please write 1-2 pages (12 pt font, single spaced) describing your occupational goals and objectives in life, and how your experience with a bleeding discorder has affected your choices.
4. Two letters of recommendation. Optional: recommenders may email letters directly to scholarships@bleedingdisordersnc.org, if preferred.
5. Current FAFSA "Submission Summary" (more at this website: Free Application for Federal Student Aid).

NOTE: If you are already enrolled in college or graduate school and your current transcript displays less than one complete year, please upload both your current transcript and your previous school's transcript. Personal Essay

Instructions:

Please compose an essay describing your occupational goals and objectives in life, and how your—or your family's—experience with a bleeding disorder has affected your choices.

Length and Format:

1 page minimum, 2 pages maximum. Please use 12 point font, single spacing, and normal margins.

Letters of Recommendation

Instructions:

Please submit two letters of recommendation (LOR). We recommend that you request at least one recommendation from an official at your school (teacher, coach, counselor, administrator, etc.).

If preferred, your recommenders may email letters to scholarships@bleedingdisordersnc.org. If they choose this option, please ask them to attach a PDF or to include their recommendation in the body of the email, and to include your name in the subject line.

FAFSA (Free Application for Federal Student Aid)

Instructions:

On the FAFSA website, select the FAFSA Submission Summary, find your Student Aid Index (SAI), and enter it below. Then, print the page to PDF or take a screen shot. We need the first page so that we can verify your name, the application date, and your SAI.
